Apply for a Library Card
Any resident, 18 years and older, of the Brentwood School District, is eligible to apply for a library card. For residents under 18 years of age, a parent or legal guardian must be present with their proof of address.
Library Card Requirements
Proof of residency or ownership of property or a business is required from one of the following and must include the applicant’s name and street address:
- New York State Driver’s License or Non-Driver’s Photo ID
- New York State Automobile Registration or Insurance Card
- Town of Islip Recreation Card
- Current Notarized Lease Agreement on official letterhead
- Current homeowner’s tax receipt
- Property Deed
- Any piece of mail with an envelope, postmarked within the last two (2) months
*Note: Post office boxes are not accepted as mailing addresses. Proof of residential or business address is required
